Effective September 1, 2025, Texas law (HB 4224) requires most health-care “covered entities” that provide services directly to patients to prominently post clear, detailed instructions both on their website and at any physical facility explaining:

How a consumer can request their health care records from the entity;

How to contact the disciplinary or licensing authority that oversees the entity; and
-
How to file a consumer complaint under the consumer-information statute (Tex. Health & Safety Code §181.103).

How to File a Complaint & Contact the State Licensing/Disciplinary Agency
If you are dissatisfied with any aspect of our work together, I encourage you to speak with me directly. I welcome the opportunity to address your concerns and make adjustments where appropriate. Open communication is a vital part of the therapeutic process.
If you believe your rights as a client have been violated or you are unable to resolve a concern directly with me, you have the right to file a formal complaint with the appropriate state licensing board based on your residency or the location where services were rendered:
